Important information regarding changes to Physical Exam requirements

Dear Parent/Guardian,

This is to inform you of important changes in New York State Education Law for physical exam requirements that will be effective July 1, 2018. 

A physical exam will be required for all students entering the district for the first time and for all children entering grades K, 1, 3, 5, 7, 9, and 11.

Some things to remember:

  • Any physical or health examination performed between September 7, 2017 and September 6, 2018 will meet this requirement. Please provide a copy of that physical examination to the health office.
  • The physical or health examination must be completed by a New York State licensed physician, physician assistant or nurse practitioner.
  • Your child(ren)’s physical or health examination form must be submitted to the school health office by Friday, October 5, 2018.   
  • If a physical or health examination form is not submitted to the school by October 5, 2018, the school will contact you.
  • The physical or health examination form can be found here; or can be sent home with your child(ren) upon request.  For the 2018-19 school year, we can also accept your child(ren)’s medical provider’s physical examination form.
  • A dental certificate is requested for grades K, 1, 3, 5, 7, 9, and 11
